Blue Cross Blue Shield of Vermont, CIGNA, MVP and Green Mountain Care (Medicaid).
Before you call to set up an appointment, please contact your insurance company to determine if you need pre-authorization to see me, require a co-pay for visits, etc. All plans are different and your insurance provider can help you understand your policy.
If you would like to see Dr. Davis but have insurance through a provider that is not listed above, she can provide you with a receipt you can submit to your insurance company for out-of-network reimbursement. In this case, you will be responsible for full coverage for the session at the time of the appointment.
